You can easily create your new SQL Report with Role Name, Created/Modified Date and Description:

Here is the query for you:

SELECT TOP 1000 [Name]
      ,[CreatedDate]
      ,[ModifiedDate]
      ,[Description]
  FROM [Symantec_CMDB].[dbo].[SecurityRole]

You can use it along with Report of Security Accounts
